The Nepal picture
Where organised screening is thin, cancers are found by symptom rather than by mammogram, and stage at diagnosis is correspondingly later.
A full breast cancer plan runs months, not days. Which parts must happen in India and which can happen at home is worth negotiating explicitly with the oncologist.
What treatment usually involves
General description. Your oncologist decides the sequence.
- 01Receptor and marker testing
ER, PR and HER2 status determine whether hormone therapy, targeted therapy, or both, belong in the plan. If this has not been done, it will be.
- 02Surgery
Breast conservation with radiotherapy, or mastectomy, with sentinel node biopsy or axillary clearance. Reconstruction, immediate or delayed, is a separate decision.
- 03Chemotherapy
Before surgery to shrink the tumour, or after it, depending on stage and biology.
- 04Radiotherapy
Standard after breast conservation and after mastectomy in defined situations. Technique and number of fractions affect both schedule and cost.
- 05Hormone or targeted therapy
Often years long, frequently continued in Nepal under a local oncologist with periodic review.

Questions Nepali families ask
Will I lose my breast?
Not necessarily — breast conservation is standard practice at large centres where the tumour size, location and biology allow it, usually combined with radiotherapy. It is a decision for the surgeon after seeing the imaging and biopsy, and it is a fair thing to ask about directly in the first consultation.
Why does the receptor report matter so much?
Because it decides whether hormone therapy and HER2-targeted drugs belong in the plan, and those are among the largest cost and duration variables in the whole treatment.
Can chemotherapy cycles be given in Nepal between visits?
Sometimes, and many families do it to reduce cost and time away. Whether it is appropriate for your regimen is the treating oncologist's call and should be written into the plan, not arranged informally.
How long is the whole treatment?
Months for the active phase, and often years for hormone therapy. Ask for the schedule in writing so you can plan travel, work and money around it.
What will it cost?
The hospital issues a written estimate after reviewing your reports. We do not publish figures.
