The Nepal picture
This is true in every country, Nepal included. Sub-specialised skull base, paediatric and functional neurosurgery is available in fewer places than general neurosurgery.
MRI is available in Kathmandu and other cities. What families most often seek across the border is a specialist opinion on whether to operate and by what approach.
What the pathway involves
General description. The neurosurgeon and neuro-oncologist decide the plan.

The tumour type is confirmed after surgery and decides whether radiotherapy or chemotherapy follows.

Questions Nepali families ask
Is my tumour operable?
Only a neurosurgeon who has looked at the images can say, and it is exactly the question a written second opinion is best at answering. Send the images and ask for the opinion in writing.
Is a benign brain tumour urgent?
Benign does not mean harmless — location and pressure effects matter more than the word. Equally, some are watched rather than operated on. The distinction is clinical, and this page cannot make it for you.
How long is recovery after brain surgery?
It varies enormously with the tumour and the patient. Ask the surgeon for an expected inpatient stay and an expected period before flying, and build buffer into your return travel.
Will there be permanent effects?
Sometimes, and an honest surgeon will discuss the specific risks for your tumour's location before you consent. Ask that this conversation happen with a family member present and, if you prefer, with Nepali-language support arranged.
What will it cost?
The hospital issues a written estimate after the imaging review. Neurosurgical estimates vary widely with complexity, so ask what the figure assumes about ICU stay.
