Symptoms of early gastric cancer are easily mistaken for ordinary indigestion. Families frequently arrive with a diagnosis already several months old, which is exactly why re-staging on arrival is standard.

What treatment usually involves

General description of how gastric cancer care is organised. Not medical advice.

    Endoscopy and biopsy review

    The histopathology, including HER2 status where relevant, is reviewed again. Blocks and slides are often re-examined.

  2. 02
    Staging

    CT of the chest, abdomen and pelvis, and in some cases diagnostic laparoscopy to check for spread that imaging misses. This step decides whether the intent is curative.

  3. 03
    Perioperative chemotherapy

    Many patients receive chemotherapy before surgery, then more after. The sequence is a tumour board decision.

  4. 04
    Surgery

    Partial or total gastrectomy with lymph node dissection. Extent of resection determines the ICU and ward stay and the nutritional recovery.

  5. 05
    Nutrition and follow-up

    Eating changes permanently after gastrectomy. Dietitian input and B12 monitoring are part of the plan, and much of it continues at home in Nepal.

Questions Nepali families ask

Is stomach cancer curable?

It depends entirely on stage at diagnosis, and no honest answer can be given without the reports. Early disease is treated with curative intent; advanced disease is treated to control it and preserve quality of life. The oncologist tells you which conversation you are in — a facilitator must not.

Why do Indian hospitals repeat scans I already had in Nepal?

Usually because staging must be done to a protocol the surgical team can act on, and because imaging ages. It is frustrating and it costs money, but a resection planned on a six-month-old scan is a worse problem.

Do I need surgery, chemotherapy, or both?

In gastric cancer the two are often combined, in a sequence decided by a tumour board. Ask whether your case will be discussed at one.

How long will we need to be in India?

It depends whether the plan is surgery, chemotherapy, or both, and whether cycles can be run in Nepal between visits. Ask the oncologist to write the schedule down so you can plan travel around it.

What will it cost?

The hospital issues its own written estimate after reading the reports. We do not publish figures, and we would rather lose the enquiry than invent one.
